What is Lupkynis?
Lupkynis is a calcineurin-inhibitor immunosuppressant medicine.Lupkynis, a prescription medication used in conjunction with other medications to treat kidney disease (active systemic lupus (SLE)), is prescribed for adults who have systemic lupus (SLE).Lupkynis shouldn't be taken together with a medication called cyclophosphamide. If you're not sure whether you should take this medication, talk to your healthcare provider.Lupkynis comes as an oval capsule in pink or orange, with VCS printed on one side in white ink. It is packaged in aluminium blisters.
Warnings
Lupkynis can increase the risk of cancers like skin cancer and lymphoma.Voclosporin affects your immune system. You could get more serious infections, including fatal ones.Lupkynis should not be taken with drugs that are known to inhibit CYP3A4, such as ketoconazole (itraconazole), clarithromycin, or ketoconazole.Inform your doctor of all the medications you are taking, as well as any new ones. Some drugs are not to be taken together, and others can cause drug interactions.
Before you take this drug
If you have an allergy to voclosporin, do not use Lupkynis.
Drug interactions can have dangerous side effects. Lupkynis should not be taken with certain drugs. If you are also taking
- Cyclophosphamide;
- Nefazodone;
- Antibacterial clarithromycin and telithromycin
- Antifungal medicine: itraconazole, ketoconazole, posaconazole, voriconazole, or
- Antiviral medicine to treat hiv or hepatitis c: boceprevir (cobicistat), dasabuvir (elvitegravir), indinavir (lopinavir) or ritonavir: nelfinavir, ombitasvir, paritaprevir, saquinavir, telaprevir, and tipranavir
If you take lupkynis along with another medicine, you should tell your doctor if you have any medical conditions and whether you are breastfeeding or pregnant.
- Heart rhythm problems
- Long-qt syndrom
- An electrolyte imbalance is a low potassium or magnesium level in the blood.
- High blood pressure
- Liver or kidney disease
Lupkynis can increase your risk of certain cancers, such as lymphoma or skin cancer. Consult your doctor to determine your risk.Voclosporin is alcohol-containing and should not be used during pregnancy. Untreated active lupus during pregnancy can cause complications, such as eclampsia. This is dangerously high blood pressure, which can cause medical problems for both mother and child. Treatment of kidney disease may be worth it if the risks to your baby are minimal.Lupkynis can be used with another medication called mycophenolate-moliferol. Mycophenolate mofetil can cause miscarriage and birth defects if used during pregnancy.Do not breastfeed for 7 days after taking your last dose of lupkynis.

How to take Lupkynis?
Follow the instructions on your prescription label and read all medication guides or instruction sheets. Read all the instructions and directions that come with your prescription. Sometimes your doctor will change the dose.Do not open, crush, chew or break the capsule. If you are having trouble swallowing the lupkynis capsules whole, tell your doctor.Take lupkynis with an empty stomach at least one hour before or two hours after eating.Dosages should be taken at intervals of 12 hours. Take your doses at least 8 hours apart.Voclosporin affects your immune system. You could get more infections, including serious or deadly infections. You will be required to undergo frequent medical tests. The results of your lupkynis treatment may cause it to be stopped or delayed.Your blood pressure needs to be checked frequently in order to remain normal.Store lupkynis at room temperature in its original container, away from moisture or heat.Lupkynis may be safe and effective for up to one year.
Details on dosage
Adult dosage for lupus nephritis :
Take 23.7 mg twice daily.
Comments:
the use of this drug with cyclophosphamide has not been proven to be safe or effective.
Use this drug with corticosteroids and mycophenolate mofetil (mmf).
Determine the actual estimated glomerular filtering rate (egfr) prior to starting treatment.
The dosage of this drug should be adjusted according to the patient's egfr.
Check the blood pressure at baseline. Do not start treatment if it is greater than 165/105 mmhg or if hypertensive emergencies are present.
If the patient has not experienced therapeutic benefit within 24 weeks, it is reasonable to consider discontinuing treatment.
Safety and efficacy have not been proven beyond the first year.
Use: as part of a background immunosuppressive regimen in the treatment of adult patients with active lupus nephritis
What happens if i miss the dose?
If you miss a dose by more than four hours, you should take the medication as soon as possible. Take only one dose at a time.
What happens if i overdose?
Call 1-800-222-1222 for poison help or seek immediate medical attention.Overdose symptoms include headache tremors and weakness, nausea, infections, or rash.
What should be avoided?
Avoid receiving a "live vaccine" while using lupkynis. It may not protect you as well from diseases, and the vaccine may not perform as well. Live vaccines are available for measles (mmr), rubella, typhoid (yellow fever), varicella, zoster, and influenza (influenza).Sunburns can be exacerbated by voclosporin. Avoid tanning beds or sunlight. When you're outdoors, wear protective clothing and apply sunscreen (spf 30) or higher.Grapefruit can interact with voclosporin, causing unwanted side effects. Use grapefruit products sparingly.
Side effects of Lupkynis
If you experience any of the following symptoms of an allergic reaction: itching; difficulty breathing; swelling in your face, lips, or throat; or hives, seek emergency medical attention.
If you experience:
- Pain or burning after urinating;
- Kidney disease: little or no urine, swelling of the feet or ankles, and feeling tired or breathless;
- High blood pressure, a severe headache, blurred eyesight, or pounding in the neck or ears.
- Nervous system problems: confusion, vision changes, headache, feeling less alert, tremors, numbness, tingling, seizures;
- High level of potassium; nausea; weakness; tingly sensation; chest pains; irregular heartbeats; or loss of mobility.
- Anemia (low red blood cells), pale skin, unusual fatigue, feeling lightheaded or short-of-breath, cold hands and/or feet
Side effects of lupin include:
- Kidney problems;
- Anaemia;
- High blood pressure
- Diarrhoea, stomach pain, and heartburn.
- Tremors;
- Mouth sores;
- Headache, tiredness;
- Painful urination
- Cough;
- Hair loss.
