Rinvoq

Drug class: antirheumatics.

Generic name: Upadacitinib.

What is Rinvoq?

Rinvoq can be used with or without other medications to treat rheumatoid arthritis in patients who have been unable to tolerate or have not responded to one or more TNF inhibitors. Rinvoq can also be used to treat psoriatic arthritiswho in adults that are not able to take or have not responded to or tolerated one or more TNF-inhibitor medications. Rinvoq can also be used to treat the symptoms of eczema in adults and children over 12 years old who are unable to use other medications to treat their condition. Rinvoq can also be used to treat colitis ulcerative in adults who are unable or have not responded to TNF inhibitor medications. Rinvoq can also be used to treat Crohn’s diseasewho in adults that are unable to or have not responded to TNF inhibitor medications. Rinvoq treats ankylosing spondylitis in adults who cannot take, or have not responded to, one or more TNF-inhibitor medications. Rinvoq can also be used to treat non-radiographic active axial spondyloarthritis in adults, but without any changes visible on X-rays. This is for those who cannot take one or more TNF-inhibitor medications or have not responded to them. Rinvoq is a member of a group of drugs called Janus kinase inhibitors.Effective treatment entails decreasing immune system activity.

Side effects of Rinvoq

If you experience any of the following symptoms of an allergic reaction after taking Rinvoq, seek immediate medical attention: hives; difficulty breathing; swelling of your lips, face, tongue, or throat.

Upadacitinib can affect your immune system and cause white blood cells to grow uncontrollably. You should call your doctor immediately if you experience:

  • Fever, swollen glands, night sweats;
  • Vomiting, diarrhea, weight loss;
  • A new lesion or mole that has changed size or color
  • You may experience speech or vision problems, as well as weakness on one side.
  • Changes in your mental state
  • Blood in urine, burning sensation when you urinate or little or no urination.
  • Tenderness around the transplanted renal

Some people who take Rinvoq develop heart attacks, strokes, or serious blood clots. Seek immediate medical attention if:

  • Heart attack symptoms: chest pain, pressure or pain that spreads to your shoulder or jaw, nausea, and sweating.
  • Signs and symptoms of stroke: sudden numbness, weakness, or slurred words, especially on one side; severe headache; problems with vision, balance, or speech.
  • Signs that a blood clot is in your lung: chest pains, sudden cough, shortness of breath, dizziness, or coughing blood.
  • Signs of a deep blood clot: pain, swelling or warmth in a leg

Rinvoq can cause serious side effects. If you experience:

  • Difficulty breathing;
  • Increased urination or pain when you urinate
  • Feeling short of breath, fever, chills, and coughing with mucus;
  • Blood tests that are abnormal
  • Signs and symptoms of tuberculosis: fever, coughing, night sweats; loss of appetite; weight loss.
  • Symptoms of shingles: flu-like symptoms or tingly or painful blistering Rash on a side of the body
  • Herpes symptoms include blisters around the mouth, skin lesions or sores, and tingling or burning pains in your lower back or thigh.
  • Signs and symptoms of an infection: fever, chills, a sore throat, body aches, unusual fatigue, loss of appetite or bruising, bleeding, etc.
  • A hole (perforation) in your digestive system—fever, severe abdominal pain, changes in bowel habits, or
  • Symptoms of hepatitis: loss of appetite, nausea, abdominal pain (upper right), dark urine and clay-colored stool, jaundice (yellowing skin or eyes).

If you experience certain side effects, your treatment may be delayed.

Some of the common side effects associated with Rinvoq include:

  • Flu symptoms
  • Nausea;
  • Acne, rash, headache;
  • Stomach pain;
  • Weight gain
  • Muscle pain;
  • Folliculitis is the redness or crusting of the hair follicles.
  • Cold symptoms like stuffy noses, sneezing, and sore throats;
  • Low white blood cell counts—fever, mouth ulcers, skin ulcers, sore throats, coughs; or
  • Anemia (low red blood cells), pale skin, fatigue, feeling lightheaded or short-of-breath, cold hands and/or feet

Warnings

Rinvoq can increase your risk for life-threatening medical conditions, including serious infections, a hole in your digestive system, a stroke or heart attack, blood clots, or cancer. If you experiencecough, symptoms such as a fever or night sweats, constant fatigue, stomach pain or diarrhea, changes in bowel habits, difficulty breathing, wheezing, or a severe cough; or if there are lumps on your neck, in your armpits, or in your groin, call your doctor immediately. Seek immediate medical attention if your symptoms includebreath shortness of breathe, chest pain that spreads to your arms or neck, nausea, dizziness or cold sweat on one side, weakness in your body or speech, pain, redness, or swelling of an arm or a leg. Worsenhepatitis B can come back or worsen, if it's treated.  Speak to your doctor if your stomach hurts on the right side, a loss of appetite is present, or if your skin or eyes are yellow.

Before you take this drug

Rinvoq should not be taken if you have an allergy to upadacitinib. Tell your doctor before you begin taking Rinvoq if you experience any symptoms of infection, such as fever or sweating. Also, you should tell them if you are experiencing body aches or fatigue, sores on the skin, or open wounds.

Tell your doctor about any of the following to ensure that Rinvoq will be safe for you:

  • A weak immune system;
  • An active or chronic infection
  • Tuberculosis, or if someone in your family has it,
  • Herpes Zoster
  • Any type of cancer
  • Diverticulitis is an ulcer on your stomach or intestines.
  • Heart problems include a heart attack, stroke, or blood clot.
  • High-cholesterol
  • Chronic lung disease;
  • Eye problems (especially those involving your retina);
  • Diabetes;
  • Low red or white blood cell counts
  • Recently received or scheduled for any vaccine
  • Are you a smoker, or have you been a smoker in the past?
  • Liver disease (especially hepatitis A and C)
  • Kidney disease is a serious condition.

Rinvoq should only be used if you have received all your vaccines.

Ofrinvoq can increase your risk for certain cancers, such as lymphoma, lung cancer, and skin cancer. You should speak to your doctor regarding this possibility. Before starting the treatment, you may need a negative pregnancy result. Can harm an unborn child. Use an effective contraceptive during the use of Rinvoq and for at least four weeks after you have taken your last dose. Inform your doctor immediately if you fall pregnant. Do not breastfeed for 6 days after taking this medication.

How to take Rinvoq?

Follow your doctor's instructions and take Rinvoq only as directed. Read and follow all instructions listed on your prescription label. Rinvoq can be taken with or without food and is typically taken once per day. Do not chew or crush the tablet. If you find part of a Rinvoq tablet, call your doctor. Rinvoq may require frequent medical testing before and during treatment. Keep this medicine at room temperature, away from heat and moisture.

Dosing information

Adult dose for rheumatoid arthritis:

15 mg once daily orally
Comments:
This drug can be used either as a monotherapy or combined with methotrexate and other nonbiologic disease-modifying antirheumatic (DMARD) drugs.
This drug is not recommended for patients with a lymphocyte count less than 500 cells/mm3, a neutrophil count less than 1,000 cells/mm3, and a hemoglobin level less than 8 g/dl.
Limitations: This drug should not be used in conjunction with other Janus kinase inhibitors (JAK), biologic dmards, or powerful immunosuppressants like azathioprine or cyclosporine.
Use: treating adult patients with moderately active to severely active rheumatoid (RA) when one or more TNF blockers have not been effective or are intolerable.

Adult dose for psoriatic arthritis:

Take 15 mg once daily.
Comments:
Limitations: It is not recommended that you use this drug in conjunction with other JAK inhibitors, biologic dmards, or powerful immunosuppressants like azathioprine or cyclosporine.
Adults with active psoriatic arthritic disease who have an intolerance or inadequate response to TNF-blocking agents

Usual Adult Dose for Atopic Dermatitis:

Initial dose: 15 mg once daily.
Maximum dose: 30mg orally, once daily if there is no response to the initial dose.
Duration of treatment: Stop if you do not see a response with the 30 mg/day dosage.

Usual dose for atopic dermatitis in the elderly:

Adults 65 and older: 15mg orally, once daily.

The usual pediatric dose for atopic dermatitis:

Patients aged 12 years and up who weigh at least 40kg:
Initial dose: 15 mg once daily.
Maintenance dose: 30mg orally, once daily (if there is no response to the initial dose).
Maximum dose: 30mg orally, once daily.
Duration of treatment: Stop if the 30 mg dose does not produce a response.
Comments:
Use the lowest dose possible to maintain the response.
Limitations: It is not recommended that you use this drug in conjunction with other JAK inhibitors, biologic immunomodulators, or other immunosuppressants.
Use: Patients older than 12 and weighing 40 kg or more with moderate to severe atopic skin disease that is not adequately controlled by other systemic drugs, including biologics. Or when other treatments are not recommended.

What happens if I miss the dose?

If you are almost due for your next dose, skip the missed one. Never take two doses of a medicine at the same time.

What happens if I overdose?

Contact 1-800-222-1222 immediately in case of poison exposure and seek immediate medical care.

What should be avoided?

You could get a serious infection if you receive a "live vaccine". Live vaccines are measles mumps, Rubella (MMR), rotavirus, typhoid (yellow fever), typhoid (typhoid), varicella (chickenpox), and zoster (shingles). Rinvoq can increase your skin cancer risk. Avoid tanning or sunlight. When you're outdoors, wear protective clothing and use sunblock with SPF 30 or higher. Grapefruit can interact with upadacitinib, causing side effects. Avoid consuming grapefruit products.

Interaction with other drug

Take caution if taking multiple medications at once; some could alter blood levels of others you are already taking and increase side effects and reduce effectiveness, thus becoming less efficient over time. Notify your healthcare provider of all of the drugs you take at once to stay safe. Upadacitinib can be affected by many drugs, including:

  • Phenytoin;
  • An antibacterial: clarithromycin and rifampin.
  • Antifungal medicine: itraconazole, ketoconazole, posaconazole, and voriconazole;
  • Drugs That Weaken the Immune System: Azathioprine, Cyclosporine and Others
  • Nsaids are nonsteroidal anti-inflammatory medications, such as aspirin (Advil), ibuprofen, naproxen, diclofenac (Indomethacin), meloxicam (Aleve), and celecoxib.
  • Steroid Medicine: Dexamethasone, Prednisone, and Others
