What is Hydrochlorothiazide  and telmisartan?

Hydrochlorothiazide works as a diuretic. Telmisartan works as an angiotensin I receptor-blocking agent (also called an ARB blocker).Combination medicine Hydrochlorothiazide and telmisartan treat hypertension. Reduced blood pressure can reduce your risk of stroke and heart attacks.This medication guide does not list all possible uses of hydrochlorothiazide or telmisartan.

Side effects of Hydrochlorothiazide and telmisartan

If you experience any of the following signs of an allergic reaction: hives, difficulty breathing or any form of swelling on lips, face, tongue or throat - seek medical help immediately!In rare cases, hydrochlorothiazide or telmisartan may cause a condition resulting in skeletal muscle breakdown, which can lead to renal failure. If you experience unexplained muscular pain, tenderness, or weakness, call your doctor immediately, especially if there is also a fever or dark-colored urine.

Call your doctor immediately if you:

  • Eye pain and vision problems
  • A feeling of lightheadedness, as if you could pass out.
  • Jaundice
  • Unusual weight gain; swelling of the hands or feet;
  • Easy bruising and unusual bleeding
  • Signs of electrolyte depletion include: dryness of mouth; increased thirst; drowsiness; confusion; restlessness; and nausea. Other symptoms are muscle pain, weakness, low energy, rapid heartbeats, and little or no urine.

Some of the common side effects associated with hydrochlorothiazide or telmisartan include:

  • Feeling tired and dizzy
  • Nausea, diarrhea;
  • Backache
  • Cold or flu symptoms

Warnings

Use it only if you're not pregnant. If you become pregnant, stop using the medication and immediately tell your doctor.This medicine should not be used if you cannot urinate.Do not take hydrochlorothiazide or telmisartan if you have diabetes.

Before you take this drug

If you have an allergy to hydrochlorothiazide or telmisartan and are unable to urinate, you should avoid using hydrochlorothiazide.Do not take telmisartan and hydrochlorothiazide together if you have diabetes.If you have kidney problems, you may need to avoid using aliskiren with hydrochlorothiazide or telmisartan.

Discuss with your physician any of the following:

  • Kidney disease
  • Liver disease
  • Glaucoma;
  • Low or high magnesium or potassium levels in your blood
  • Allergies or asthma
  • Lupus;
  • Diabetes
  • An allergy is a reaction to sulfa drugs or penicillin.

If you are pregnant, do not take this medicine.. If you are pregnant, stop using the medication and call your doctor immediately.This medicine should not be used while breastfeeding.

How to take Hydrochlorothiazide and telmisartan?

Read all the instructions or guides that come with your medication and follow the directions. Sometimes, your doctor will change the dose. You must take the medication exactly as prescribed.

This medication may be taken with or without food.You will need to check your blood pressure often.If you have diarrhea or vomiting or are perspiring more often than normal, call your doctor. It is easy to become dehydrated when taking hydrochlorothiazide with telmisartan. This can cause very low blood pressure, a serious electrolyte imbalance, kidney failure, or severe low blood pressure .Tell your doctor if you plan to undergo surgery or any medical tests that you will be using this medication.This medication may take 4 weeks to bring down your blood pressure. Continue to take this medication as prescribed, even if your symptoms are normal. High blood pressure is often asymptomatic. It is possible that you will need to take blood pressure medication for the rest of your life.Store at room temperature, away from moisture or heat. Keep the blister packs until you're ready to use each tablet. Remove the tablet by gently pushing it through the foil.

Details on dosage

Usual Adult Dose for Hypertension:

Hydrochlorothiazide, 12,5 to 25 mg; Telmisartan, 80 mg once daily.
Maximum dose: hydrochlorothiazide 25, mg-Telmisartan, 160 mg orally, one time a day.
Comments:
This drug can be used in place of the individual components.
If blood pressure is not controlled by hydrochlorothiazide 25, 25 mg, orally one time a day, you may switch to hydrochlorothiazide 12, 5 to 25 mg of telmisartan (80 mg) orally every day. Reassessment after 2 to 4 weeks: you may be able to increase the dose up to 25 mg of hydrochlorothiazide-telmisartan orally, once daily.
If the blood pressure is controlled with hydrochlorothiazide 25 mg once daily monotherapy but hypokalemia occurs, then you can switch to hydrochlorothiazide 12,5 mg and telmisartan 80 mg once daily.
If blood pressure is not controlled by the telmisartan monotherapy of 80 mg once daily, you can switch to a combination of hydrochlorothiazide (12 mg) and 80 mg once daily. You may also titrate hydrochlorothiazide at 25 mg and 160 mg once a day, if needed.
Use: hypertension treatment; not recommended as a first-line therapy.

What happens if I miss the dose?

If you are almost due for your next dose, skip the missed one. Never take two doses of the same medicine at once.

What happens if I overdose?

What should be avoided?

The use of hydrochlorothiazide can increase your risk of skin cancer. Avoid tanning beds or sunlight. When you're outdoors, wear protective clothing and use sunblock with an SPF of 30 or more. You may be asked to undergo regular skin checks by your doctor.Use potassium supplements and salt substitutes only if your doctor recommends them.Alcohol can lower blood pressure further and cause adverse effects.Avoid overheating or dehydration during exercise, in hot weather, or by drinking too little fluid.

Interaction with other drug

Avoid taking colestipol or cholestyramine within 4 hours of hydrochlorothiazide or telmisartan.

Tell your doctor all of your other medications, including:

  • Any other heart or blood-pressure medications
  • Digoxin;
  • Lithium;
  • A diuretic, or "water pill".
  • Insulin, or oral diabetes medication;
  • Opioid pain medicine;
  • A sleeping pill.
  • Steroid medication or adrenocorticotropic hormone (ACTH);
  • A barbiturate, such as butabarbital, phenobarbital, or secobarbital.
  • Aspirin, Ibuprofen, Naproxen, Diclofenac, and Indomethacin are all nsaids.
