What is Dojolvi?

Dojolvi, a synthetic triglyceride with three odd-chain molecules of fatty acids, is a medium-chain molecule.

Dojolvi, a prescription medication, is used to treat disorders of long-chain fatty acid oxidation in both adults and children. Long-chain fatty acid oxidation disorders (LCFAOD) are rare genetic disorders that affect the body's capacity to convert certain fats into energy. Low blood sugar can result in muscle weakness, heart issues, and life-threatening medical conditions.

Warnings

Tell your healthcare provider if, before taking Dojolvi, you are taking a lipase inhibitor such as orlistat (Xenical), because it could affect the effectiveness of Dojolvi. Please follow all instructions on the label or package of your medication. Inform your healthcare provider about your medical conditions and allergies. Also, tell them which medicines you are using.

Before you take this drug

Tell your doctor about any problems you've had with your pancreas. Dojolvi might not work well if you have a pancreas problem. Inform your doctor immediately if you become pregnant or nursing. You may be asked to participate in a study on pregnancy safety that tracks the effects of triheptanoin.

How to take Dojolvi?

Dojolvi should be taken exactly as prescribed by a doctor. Read all the instructions and directions that come with your prescription. Sometimes your doctor will change the dose. Dojolvi should not be taken if you are currently taking any other medium-chain triglyceride products. Dojolvi should be taken with food or as a snack at least four times a day. Do not forget to follow your doctor's instructions.

Dojolvi is best taken with liquids or soft foods. Take the medicine with food or liquids. Mix this medicine with:

  • Fat-free yogurt, plain or flavored
  • Fat-free puddings or smoothies
  • Low-sugar applesauce;
  • Fat-free cottage cheese;
  • Skim (fat-free) milk;
  • Baby food that is fat-free
  • Whole-grain hot cereal

Follow all instructions for mixing Dojolvi with liquids or soft food, or how to mix the medicine and administer it through a feeding tub. If you don't understand the instructions, ask your pharmacist or doctor. Mix the medicine in an empty, clean container. Dojolvi should not be mixed or stored in any container, syringe, or cup made from polystyrene or polyvinylchloride (PVC). This medicine should not be administered through a PVC feeding tube. If you continue to use Dojolvi, your feeding tube may develop problems. Make sure you check your feeding tube regularly to ensure it is functioning properly. Dojolvi may require you to adhere to a special diet.Please follow the advice of your physician or dietitian.Learn what foods you should eat and avoid to control your condition. You may need to keep track of the calories you consume every day while taking Dojolvi. Dojolvi should be stored in its original container at room temperature and away from moisture. Do not freeze. Use the mixed medicine within 24 hours. Store it in the fridge. Dojolvi should be thrown away 90 days after opening the bottle, or at the date of expiration.

Dojolvi adult dose for functional gastric disorder:

The recommended total daily dose (mL) is calculated as follows: Patients DCI (kcal) * target % dose DCI/ 8.3 kcal/mL
Round to the nearest whole number
Divide the total daily dose by four roughly equal doses.
Dose Initiation/Titration:
Patients who are not taking medium-chain triglyceride products:
Start at a dose of approximately 10% DCI, divided into 4 separate doses.
Increase the recommended daily total dose over a period of 2 to 3 weeks.
Patients who switch from another MCT product
Discontinue any previous MCT prior to starting therapy.
Start with the last dose of MCT tolerated, divided into 4 separate doses.
Increase the total daily dose by approximately 5% of DCI every two to three days until the target dosage is reached.
Comments:
Calculate doses after calculating daily caloric requirements (DCI).
The target for DCI is recommended at 35%.
This product has a caloric value of 8.3 kcal/mL.

What happens if I miss the dose?

If you are almost due for your next dose, skip the missed medicine. Take the remaining doses of the day every 3–4 hours. If you can't take the four doses in one day, skip the one that was missed. Never take two doses in one go.

What happens if I overdose?

Call 1-800-222-1222 for poison help or seek immediate medical attention.

What should be avoided?

If your doctor prescribes any restrictions regarding food, beverage, or activity, follow their instructions.

Side effects of Dojolvi

If you experience any of the following symptoms of an allergic reaction: hives, difficulty breathing, swelling of your lips, face, tongue, or throat, contact emergency medical assistance. Dojolvi can cause a variety of side effects.

  • Nausea, vomiting;
  • Stomach pain;
  • Diarrhea.

Interaction with other drug

Tell your doctor about your other medications, including:

  • Orlistat (alli, Xenical).
