Fosrenol

Generic name: lanthanum carbonate [LAN-than-um-CAR-bo-nayt]
Drug class: phosphate binder

What is Fosrenol?

Fosrenol stops the body from absorbing the phosphate. This allows it to be excreted from the body. Fosrenol can be used to lower the phosphate level in patients with end-stage renal disease. A high phosphate level can prevent your body from absorbing calcium. This can lead to serious medical issues. Fosrenol can be used in other ways not mentioned in this guide.

Warnings

Fosrenol should not be used if you suffer from severe constipation or a bowel obstruction. Tell your doctor before taking Fosrenol if you suffer from a stomach or intestinal ulcer, ulcerative colitis, diabetes, bowel obstructions of any kind, colon cancer, or have had surgery on the intestines. Chew the chewable tablet completely before swallowing it. Take this medication with food or within 30 minutes of eating. Inform your doctor of all the other medications you take, including antibiotics, blood pressure or heart medication, blood thinners, and seizure medication. Take your thyroid medication at least two hours before or after you take fosrenol. You should not take lanthanum and thyroid medication at the same time. Avoid taking an antiacid 2 hours before or following the time you take lanthanum. Some antacids may make it more difficult for your body to absorb lanthanum.

Before you take this drug

Fosrenol should not be used if:

  • Bowel obstruction, or severe constipation.

Tell your doctor about any of the following to ensure that fosrenol will be safe for you:

  • A stomach ulcer;
  • Any type of obstruction in the bowel;
  • Slow digestion or severe constipation
  • Surgery on the stomach or intestines
  • Diverticulitis, ulcerative colitis, and crohn's disease
  • Cancer of the stomach or intestine
  • Peritonitis is an inflammation of the membrane that lines the organs in your abdomen.

Tell your doctor if you plan to undergo an x-ray, CT scan, or other type of imaging of the stomach. Unknown is whether lanthanum can harm a baby in utero. If you plan to get pregnant or are already pregnant, tell your doctor. There is no way to know if lanthanum can pass into breastmilk or harm a baby who is nursing. Inform your doctor if breastfeeding is a possibility.

How to take Fosrenol?

Follow the directions on your prescription label to take Fosrenol. Please follow all the instructions on the prescription label. Your doctor may change your dosage from time to time in order to get you the best results. Use this medication only as directed. Do not exceed the recommended dose or use it for longer. Take fosrenol immediately after or with food. You must chew or crush the chewable tablet completely before swallowing it. Do not swallow the tablet whole. If you are having difficulty chewing, tell your doctor. Oral powder does not dissolve in liquids. Sprinkle a little powder on applesauce or any other soft food. Do not chew. Swallow immediately. Don't save the mixture to use later. You may need to have frequent blood tests while using fosrenol. Store away from heat and moisture at room temperature.

Details on dosage

Adult dose for hyperphosphatemia and renal failure:
Initial dose: 1500mg orally daily, divided into two doses.
The serum phosphate levels can be adjusted by titrating in increments of up to 750 mg/day for 2 to 3 weeks until the desired level is achieved.
Maximum dose: 4500 mg/day
Comment: The majority of patients will need daily doses between 1.5 and 3000mg to reduce plasma phosphate to less than 6 mg/dL.
Use: Reduce serum phosphate in patients with end-stage renal disease.

What happens if I miss the dose?

As soon as you recall, take the missed dose. If your next scheduled dosage is approaching, skip the missed dose. You should not take more medicine to compensate for a missed dose.

What should be avoided?

Fosrenol may make it difficult for you to absorb any other medications that you take orally.

  • Take your thyroid medication at least 2 hours prior to or 2 hours following the time you take Fosrenol.
  • Take lanthanum CO2 for at least 4 hours before taking an antibiotic like ciprofloxacin or levofloxacin.
  • Take fosrenol at least 2 hours prior to or 2 hours following an antacid.

Avoid taking other medicines by mouth several hours before or following the lanthanum.

Side effects of Fosrenol

If you experience any of the following symptoms of an allergy to Fosrenol, seek immediate medical attention: hives, difficulty breathing, swelling of the face, lips, or tongue, or swelling of your throat.

If you experience:

  • Constipation severe;
  • Severe stomach cramps.

Fosrenol can cause a variety of side effects.

  • Upset stomach, stomach pain, nausea, vomiting;
  • Constipation;
  • Itching, rash;
  • The tablet can cause tooth damage if it is chewed.

Interaction with other drug

Lanthanum can interfere with the absorption of other medications, making them less effective. Do not take fosrenol with any of these medicines.

  • Thyroid medications: Take at least 2 hours prior to or 2 hours following the lanthanum.
  • Take an antibiotic like ciprofloxacin or levofloxacin at least one hour before or four hours after taking lanthanum.
  • Take an antacid at least 2 hours prior to or 2 hours following the lanthanum carbonate.

Other drugs, such as prescription and over-the-counter medicines, herbal products, and vitamins, may also affect lanthanum. Inform your doctor of all the medicines you are currently taking and those that you will stop or start using.
